At AZBSC, our services are provided on a fee‑for‑service basis. We participate with a number of major health‑insurance plans and will gladly submit claims on your behalf when we are an in‑network provider. If your health plan does not list AZBSC as a participating provider, or if you prefer not to use insurance, you are welcome to self‑pay for your care. We encourage all patients to contact their insurance carrier or our office in advance to confirm coverage and discuss any out‑of‑pocket costs.
